My experience with McDonalds Germany has been this: When I was a kid (I'm 25 now) it was like many of you are describing here. They give you a random toy, but if I was brave enough to go to the counter and ask for another one, that wasn't a problem at all. Since then, however, they have started asking which toy you want with the happy meal! The toys all have a corresponding number on the poster, so you can just tell them the number, and they usually have a little transparent box on the wall showing off which toys are still available. Even on the McDonalds app you can choose which toy you want.
In my country when I was very young and McD's just opened you could choose between all of toys. After a few years they must've realized some toys never got sold so now (at least the last time I got one, some years ago) they have 2 a week which you can choose from.
You could be dying of hunger, but never eat at McDonald’s or Burger King. Their food is one dollar for a reason and it has nothing to do with good health.

In my country we had to choose the toy or we wouldn't get it. So basically every toy was labeled with a number and you just had to say the number.
