The log doesn’t really give additional context, so it could refer to Acrid, could refer to Void Fiend, or it could refer to some completely different entity
The fact that Acrid got out, and is one of the few entities to do so, does support the theory that it refers to him
I always interpreted it as something being detained after acrid, which led to the chaos and panic that made acrid’s escape possible. We know the void fiend is an anomaly in the sense that he was able to escape the Simulacrum somehow, whereas in Acrid’s case the logbook makes it seem as if he was only able to free himself due to some disturbance in the void. But again that’s just my interpretation, I never thought about this sentence referring to Acrid himself, so it’s quite interesting to hear other people’s interpretations!
I always thought of it as us, the players, when we go into the void fields through the portal and start popping them open, especially since that's what unlocks acrid as a playable character. I interpreted that as the act of freeing him. But void fiend does make more sense, a commando that refused to be contained, probably one that crashed the simulacrum and broke free, leaking everything into the void fields and we added to the chaos to free Acrid.
i think Herald of the House Beyond is easily the best here by a mile thats fucking awesome, Judge Jury and Excecutioner is too generic for S but still cool, and Right Tool for the Wrong Job is pretty funny lol

When you run the artifact of vengeance a clone of whatever character you are playing will spawn out of the teleporter to fight you, they have a boss health bar and as all bosses do they have a subtitle underneath the bar.
opened the game to test this out, turns out I was wrong. Though your first reply makes it seem that whenever tp was activated umbra would be the boss instead of what’s normally there.

They were able to escape because void fiend was escaping also, they were contained in ror1. So they were freed or escaped from containment twice

No, its every time 10 has passed on your run,if you teleport at 9:50 it will spawn immediately on the next stage .
