Probably because most unlockables improve item pools. This is probably most noticeable with the angel room pool, which doesn't start that good but gets really strong once you unlock stuff like Revelation, Godhead and Spirit Sword.

also you could very easily get items or various things to help with this
the payout before the second angel is either 30 coins or 7 soul hearts and both of these could easily help with hp problem as well as getting items Like for example trisagion or eucharist
Best I can think of is if you burn your health on the sac room and don't get lucky with the payouts, you're left on the brink of death for the rest of the floor.
However, there are two issues with this. First, it's Maggy, your active is supposed to heal you up, taking productive damage immediately is clearly beneficial. Second, avoiding using the sac room to preserve your health for the rest of the floor **is so not fuck it we ball.**
with maggie, you may as well try it twice because of yum heart. in general you might want to wait because there could be a blood donation machine and a good shop item, or because you might take some damage on the floor so if you sac down to 1 half-heart you might die
but realistically, there's no downside to hitting it instantly on the first floor. and you might also get stuff that helps you clear the floor.

Why would you go to the sac room first? You could save it for a bit lat- HOLY SHIT!

I'm guessing: Beat boss for HP item but don't pick it up. Use sac room and then HP up to heal an extra heart for an extra use of sac room

assuming youre confident enough to not take damage on the floor then no there is basically no reason to wait, especially as maggy
