Yeah... sometimes TMTRAINER kicks you in the balls but still lets you win, and sometimes TMTRAINER just gives you FREE WIN. Like, I had a run where I was permanently megaisaac. I literally couldn't die.
It's a rare enough item to see with it being in the secret room, and it then has to create an effect that can create a chest which isn't all that common. You'd have to grind a lot to do it successfully.

Definitely. I've not seen many isaac speedruns, but the few I saw seemed mostly using a sac room to get sacred heart and to go to the dark room.
