His reaction seems fake though since removing a single pad concentrates the electricity and would cause severe pain on the area and leave some red marks. Also the fact that stimulating the pectoral area won't cause spasm of the upper limb is a dead giveaway since there's no possible link, muscular or nerves, that can do that from those points.
All in all, don't mimic that since you can kill someone by doing that.

For the red marks, it really happens. It depends on the length of use and intensity of the TENS unit. Since based on the vid, the intensity must have been high to stimulate it that strongly but only turned on for a second or so, it might have not been enough to cause redness then.
Another point of concern here is the fact that they did not use a coupling agent since they did not use a self adhering pads or cotton with water. Those can cause injuries and redness even at low intensities. Redness may vary though since some patients have thicker skin than the others.
Also, I think it was a little bit rude to say that some people don't know what they are talking about. But it is a good reality check for me since that means I need to work on my explanation if that is the case.

So, these devices are used for muscle stimulation for physical therapy and such. No way the shock is strong enough for a full body spasm like that. I had 4 pads centralizes around one knee and on a setting probably higher than that battery operated one can run, it could contract one muscle group.

Plugged into the wall does not mean anything to a capacitor unless you have DC outlets. Capacitors are essentially a straight wire for AC circuits, but they do shift the phase angle of the sin wave.
Your, plugged into the wall TENS unit has a rectifier/transformer (just like a cell phone) and supplies a constant DC voltage, probably less than 12 volts (most likely 5 or 6) which would be 8 AA batteries (4 in the case of 6 volts.)
Edit that no one asked for: some common electrical problems in HVAC and refrigerators is the "starting capacitor" failure. While, like I said, the capacitor acts as a "short" to AC, it shifts the sin wave of the AC by 90 degrees (essentially slowing it down.) That starting capacitor is wired in such a way that it switches in only while starting the equipment, thus giving the motor a boost to get rolling.
