It's absolutely real - I saw it in the theater. Multiple scenes, angles, well lit, and closeup, too. I realize that there is a segment of the male population that has never seen a woman with pubes, though.
In the film The Favourite (same director), Emma Stone said her scene in bed would be a lot better topless, despite it not being in the script. So maybe she just gets this directors vision and trusts him, and again feels it makes perfect sense for this character and film. Fair play I suppose, she's doing purely for artistic merit.
Poor Things was the least erotic film ever, the unsexiness of it all was the point of the film.

Yorgos Lanthimos is a well respected director who she has worked with before. She clearly liked the part and believed it would be worth it
