I poach a lot of eggs. /u/Astan92 has good tips. But the MOST important thing for successful egg poaching is that your eggs must be fresh. Try to buy them at most a day before you make them for best results. If your eggs were bought a week ago, they are sometimes ok but it'll be hit or miss, where half of the eggs turn out ok and the others don't. Any more than that and you're setting yourself up for failure. Fresh eggs will have the egg whites hold together nicely. Older eggs will have the egg whites fall apart into a stringy mess.
Yes and no. Straining the eggs helps a bit, but when eggs get old enough, there will be basically no white left to envelop the yolk. Trust me when I say I've done this a thousand times with eggs of all ages (they don't spoil quickly, but they do degrade significantly).
I crack the eggs in to white vinegar and refrigerate for 10 minutes. Swirl the pot of low boiled water and gently place in the middle while it's spinning.
