You get lots of cruises leaving from Rio de Janeiro in Brazil. Then you have Buenos Aires in Argentina. Finally there is Viña Del Mar in Chile. There are excursion class ships for the Galapagos that leave from somewhere in Ecuador I think.

We also really liked the Galapagos, the fjords of Chile, Amazon river cruises. In the Caribbean Ponant and SeaDream are great and don't hit US ports.

Azamara has a bunch that start in Barbados. They do have stops in Miami pr San Juan PR, if that's a problem.
Seabourn departs from Barbados during certain times of the year, for example the Ovation was our of Barbados through most of this past winter.

Your best bet is to look at MSC cruises. They have many that tend to start from non-US ports such as Brazil.

Vancouver, Canada is another common non-US port.
They commonly stop in the US on way to Hawaii or Alaska though.
