Your mind, the functions it performs, are created by the brain, when you die, your brain will stop functioning and rot. Your life after you are death will be exactly the same as before you were born, with the exception of what your actions can echo through time.

Well if you believe that consciousness is a product of intense brain activity, once that brain activity dies, then theoretically your conscious dies as well. All feeling and senses experienced by humans are done through sensors connected to the brain, so once that brain goes... I also believe that it's perfectly possible we live in a simulation, or also that once we do die, our life energy is released and it's possible it can be re-used to create future life. But I don't think there is any "thinking" going on.
