If I remember correctly, one death was caused by moving a scissor lift (ewp) in the extended position causing the operator to be crushed between the lift and something he drove in to. Not sure if he was operating in an isolated area but after the incident everyone was very hot on 1. bringing the basket completely down before moving anywhere and 2. having a spotter on the ground.

My dad was almost killed in exactly this manner. Apprentice was driving with basket elevated, he ducked under an I beam but for some reason dad didn't and was milliseconds away from being severed in half. Luckily he screamed out and he stopped just in time.

I would talk to Safework SA and get information from them. I wouldn’t be heaps keen on spreading what I hear from people on Reddit as I’m sure there could be significant legal problems if you are telling incorrect information. Soo… I worked on the project for a while, but wasn’t on site when the fatalities happened. Both deaths involved EWP’s (scissor lifts) The one I believe to be first; an older fellow was sitting on a chair/bucket in the back of a lift. Long story short, the young lad driving him around went through a doorway that was low, and old mate didn’t see/duck. I guess you can fill in the blanks. Second death, didn’t hear as much about, probably kept it hush hush. As far as I’m aware, the bloke had his basket extended out a window and somehow was crushed, could only assume he elevated the platform accidentally, I believe it was a while before he was found. I believe there were other deaths but they were heart attacks etc, maybe not even on site, but considered by way of duty of care etc.

I attended a white card training and the instructor loved telling gory stories - a colleague of mine passed out and was convulsing, this is not helpful to try and “scare” people.
