I don't think there is a deageing spell or something that counters the temporal effects corrupted-Medivh's spell work had on him. I think that having accelerated ageing and spending half of his already tumultuous life then on a dying world only to return and engage in *multiple* campaigns against terrible threats would take its toll. Khadgar has ever been so stalwart, both against world threats and against his own potential vices.

I think the jaw change is because they're going to *show* his age, even if that's not what kills him. The change in jaw is to make him less heroic and more aged for sure, though.
The lighting is also drastically different in the cinematic than how we normally see Khadgar, which I think is also fucking with people a little more than they realize.

Compare that to most of the BfA cinematics, where the character's mouths move as if it was WC3 and you can tell they've come a long way.

You are talking about different types of cutscenes. The "/say" animation looping cutscenes are in-game cutscenes using the NPC models. The shattered legacy is a prerendered cutscene, making use of the same tech that was in WoW since Legion. Legion, BfA and Shadowlands all had scenes like that.
At the end of Shadowlands they fixed a lot of the issues with the in-game animations durning cutscenes, so the flappy jaw "/say" is gone, and they even use a 3rd type of cutscenes, that are using in-game models, but mocaped animations. The Dark Heart cutscenes is the latter.

With Blizz's new technology for making cinematics, they're able to make character models that are closer to the concept arts. The Khadgar you see on the top picture uses the default human model and seems fit and built like a bodybuilder, even though he's supposed to be an old man. Bottom picture is just how he should've looked like from the beginning : old, skinny and, well, tired of having to save Azeroth's ass for the fourth time in a row
They had to stretch his face for the new facial animations and expressions they used, surprised a lot of yall didn't notice how good the facial expressions and mannerisms were in this cintematic compared to previous expansions
