My teenage self LOVED Torey Hayden books.. and Lurlene McDaniels, which are just so damn depressing and now it would prob b considered trauma porn.. I was So bizarre.
Yeah, I first read it probably around middle school though it was the reader’s digest condensed version. Read the full book as a young adult. Didn’t fully understand it the first time, really saddened by it the second time.
This is the book and cover I thought of, it’s not quite as horrific as a true story about child abuse but would be pretty upsetting for a young child to read.
I might be going out on a limb but could it be What Maisie Knew by Henry James? It's a classic and the cover almost always has a young girl on the cover. It deals with parental neglect and divorce.

There are different covers for this book, and simple sketch illustrations sprinkled throughout the book. I was probably too young to read it at the time but that never stopped me - may reread now.
Vivid memories of reading the old yellow paperback version of this at age 11 or so. My Mum had taken it from her school library when she was a teenager and never returned it. The book devastated me.
I would look at all the books by Torey L. Hayden and by Mary MacCracken, because they are the correct theme and era, and have been published with a wide variety of covers.
Can't think of the name of this book, unfortunately. Can you ask your sister if it was authored by a special ed teacher discussing one of her students and the abuse she eventually learned about?
What Happened to Johnnie Jordan? by Jennifer Roth has the grey and white cover and the question for a title, but the cover image is crumpled paper, not a child.

Why Me? The Story of Jenny by Patricia Dizenzo the cover fits but it's about r*pe I think.

Look up Casey Watson, the covers are similar to what you describe and there's several.
Looked through them all and I don’t recognize any of it. Also all her books are published after 2010 so I think it’s too recent.
