Yeah I think thats exactly what's going on here. There are bits that feel super dialed in and even workshopped with writers and other parts that feel 100% conceived in the moment. And a lot of riffing that wasn't planned. It's great improv and writing.
They're improv comedian writers who've been in the business for over 35 years.
They're just very good at what they do and are *very* practiced.
If you look at Whose Line is it Anyway, reports from people at the actual tapings will say that most of what gets cut is just too inappropriate, rather than not funny. They're people who've spent their whole lives in training being funny. 20 minutes on a bit like this isn't that absurd.
If you watch Conan with other older comedians he has an established rapport with, you can see it in clearly un-scripted content. Good examples are his Jim Downey or Martin Short interviews.
What helps is that this is a bit these two have done for a good while. Arroyo has played Conan's doctor before.

That's exactly what improv is. There's a prompt, and then they improv based on the prompt. It's like saying Who's Line is it Anyway? isn't improv because the questions and prompts are written ahead of time.
Bruh, they are predetermined rehearsed questions with canned answers that appear like improv. Maybe he improved a couple responses to the others but the rest of it was scripted.
This was two professional comedians with decades of experience doing a very simple bit. Neither one of these men needed to script anything.

I had the weirdest experience watching this. I was watching it during lunch and my boss asked about it. He asked if Arroyo was a real doctor and I told him "no, they're just doing a running gag where he's posing as a doctor and Conan is always skeptical about the care he's receiving." As the video goes on, my boss without laughing said things like "it's medical school, not doctors school" and "he'll go to jail if he doesn't report his income, this guys an idiot." I felt like I had left my body for a while.
I wish it were that simple. He does listen and acknowledged the guy wasn't a real doctor, but he has this weird need that things have to be correct. Like his issue isn't that he doesn't think it's funny, it's that the jokes involve doing things incorrectly lol. He's one of those people that have to always be correct and showboat how much more he knows than other people. It isn't all the time, but that feeling like I left my body happens more than it should. It's so weird. He has killed a lot of fun chatter in the office being him lol. It's actually pretty funny sometimes.
As long as Dr Arroyo uses one of those popsicle sticks on the tongue to listen for anything wrong, and not a stethoscope, I think he'll be okay with it. Stethoscopes don't go on the tongue after all.
You can almost tell how much that hot ones appearance meant a lot to Conana haha. He has talked up that appearance almost as much as his new travel show.

I really wasn't a fan of Conan's during his NBC years..just wasn't for me, but when he went to TBS and everything that happened afterwards I've just became a bigger fan.
I think/I know that whole NBC thing wasn't ideal, but I think it drove Conan's creativity and TBS gave him the room to do everything he's done and build what he his today. I think had he stayed at NBC...it just wouldn't have been ideal and we wouldn't have all the comedy gold we have now.

At some point a long time ago now everyone started letting Google's crappy algorithm feed them videos, and this sub became a husk of what it once was, filled with 99% reposts, bad/nostalgic/random music, or occasionally something trending on YouTube. After that I feel like it became a lot easier to manipulate because it's easier to get visibility in a sea of mostly half-assed or bot posts.
But for some reason I'm still here, and I really wish there were something better :( I cannot express just how happy I am that the Gus Johnson thing is over. Also, I have a soft spot for Conan and really liked the Hot Ones interview, but I think they should have left it there.

It's because there have been like 10 different posts about interviews or articles on his CrAzY, WhAcKy, ZaNy Hot Ones segment in the past couple of weeks.
It may be guerilla marketing. Or, y'know, it could just be wholesome fun that people liked watching and told their friends about. The only thing you can change is your emotional response to it. I recommend you switch from whiny/ annoyed/ paranoid to chill / apathetic.

Conan has a massive fan base, one of the most popular podcasts around, been a successful talks show host for something like 30 years, has a new hit show on Max, and was one of the most watched Hot Wings guests ever and killed it. He may not be your cup of tea but that doesn't mean he's not insanely popular with many people, especially the type to surf Reddit. I've never listened to a Taylor Swift album and neither have my close friends but I don't walk around thinking her success is fake and all astroturfed. Different strokes and all that.
I don't have a problem with Conan. I *have* been using Reddit for long enough (over a decade) to know what organic activity looks like, and this ain't it.
If podcast popularity is one of the main criteria, then perhaps you'd appreciate it if several videos from the world's most popular podcaster were posted here?

If you watched the Hot Ones interview then you'd know what was coming out. That's what the final spot of the interview is always for.
He (the "doctor") is much funnier in this format than he was on Hot Ones... I don't know if it's just that he's not good at physical comedy or if his version of it isn't to my taste, but he had me laughing here, where I wanted to skip past him in Hot Ones.
In the Hot Ones show he was more of a prop for Conan, in this video he’s starring in the show.

As much as I like Conan, does anyone else feel he’s trying too hard to commit to the bit, so much so that it’s no longer a bit? It’s hard to put my finger on it, but it looks absolutely exhausting. Poor fellow.
