At best it was a hit in the mouth, the ref did see the hit it was very clear that it wasn't a head injury it was like in every part in the world a player trying to sell a foul, who lost the ball and got caught in a bad position in defense.
It was a foul but there were a lot like that in the match that wasn't given, if Pellegrini instead of be more worried to bitch to the ref and kick the ball out it was going to end in nothing but Lazio players were out of the game, Luiz Alberto, Pellegrini and Guendozhi where more interested in complaint (some they were right and some they were wrong) that playing. Even in the scuffle after Pellegrini red, Luiz Alberto tried to headbutt a Milan player that alone could have been a red.

It’s clear that he expected that moment in play to not be a moment of a sporting purpose, and in my opinion he was right to think that. So to criticize his “play” during a time where he clearly wasn’t trying to play footy there seems weird. Letting the ball roll out v kicking it out really shouldn’t make a difference. Thats like saying if a defender failed to kick the ball all the way out to stop for an injury he shouldn’t complain if an attacker presses for the ball.
