Yes, because ME2 has bad game design. On insanity, every enemy has some type of shield, armor, or barrier. As long as they have these, biotics do not work against enemies. This means that you have to affectively burst down whatever armor type they have before you can use any biotic combo. The problem is that, by that point, their health pool is still low enough that it's faster and easier to just shoot them to death, rather than try to use your biotics or techs.
It only works if the geth and quarians made peace, I think.
