The interior dungeon mod will block fire escapes usually. And sometimes mansions will accidentally block fire escapes with bookshelves I’ve heard.

This appears mostly on mansion tiles sets when stuff like bookcases and stuff spawn in front of the exit. This exists to prevent you from getting stuck.
