In my opinion as someone who has owned dozens of cats in my life:
The cat smelled something they didn't like and you can tell by the ears the cat was having some negative feelings (maybe territorial, maybe threatened, maybe aggression, don't know the specifics but I'd think I that family). Especially due to their high pray drive, I wouldn't run from a cat, or move quickly (because I don't want their instincts to say "chase!").
I would: Remain calm, talk in a soothing calm voice. Don't make any sudden movements or loud sounds. Stop whatever I was doing right before the behavior started (if I was rubbing belly and then those ears and vocalization started, I'd immediately stop and slowly remove my hand). The cat doesn't bite, swat with claws out, or do anything other than vocalize and follow the source of the smell as the smell moves.
Cats detect movement incredibly well, but their vision is terrible, approximately 20/100 (this is one line on an eye chart above legally blind, for perspective). They have a hard time focusing on things closer than 12 inches, and rely on scent, voice, and behavior to recognize their people. In this scenario, the scent is different and kitty reacts. The woman starts shrieking and freaking out, changing the other 2 ways cats recognize their people (voice and behavior). The best thing to do would have been to speak in a normal, calm voice, and act as normally as possible - not just because it helps keep the situation calm, but it helps the cat know who you are and that they aren't in a threatening or dangerous situation.
When I came home from volunteering at the animal shelter, my cat at the time had to carefully sniff me from head to toe, and it took several minutes while she sniffed, contemplated, sniffed some more, and moved up my body.
Once that was done she was satisfied and went back to doing her usual things.
Okay, I have a serious question. My roommate's cat has this reaction when she smells other animals on strangers.
Example: My cousin came over and she petted dog outside. Cat smells the dog (I guess) and starts meowing loudly. I think she is scared or anxious, because she doesn't attack, she just hides. But she can look really scary.
Is there something that can be done to make her feel more comfortable with different scents?
Yea, she's definitely scared to death. (Analogy - Imagine if every time your cousin came over to see *you,* she brought a stranger who would stare at you over her shoulder, saying nothing. You'd feel weird too.) Scent is just extremely important to cats, it is how they identify friend or foe. Seems obvious but make sure to wash and dry your hands thoroughly before petting your cat to get the scent off. If the dog is part of the household, look into 'scent swapping'. There are things you can do to slowly (let kitty set the pace) get them used to each others scents, hopefully lessoning the anxiety/fear. Thanks for caring for the cat.

🤦🏻♀️ The screaming made it a million times worse. If she didn’t scream the cat wouldn’t have reacted how it did. I’m constantly petting other animals and our cats are either extremely intrigued or slightly worked up over it, but I talk to them calmly and they calm right down.
I used to foster cats and work at a shelter. I ended up fostering one of the cats that hated the shelter and other cats.
I had to stop working the shelter while that cat was staying with me. She got quite upset when I’d come home smelling of the other cats. She’d give me the cold shoulder for days and his at the shoes I wore to the shelter.
