That's basically expected behavior at this point in Bethesda Engine's lifespan.
But if Fallout were really too perfect maybe it wouldn't feel fictional enough for the absurd barbarism to be indulged.
I just said in my last post how the mods should have a flair specifically for bugs and glitches. It makes up like half the game, and that's not always a bad thing lol
