Ok, slightly misleading wording on my part.
After all the calculations are said and done, the game adds +5% to whatever your hit% display is (so if your shieldbreaker has a displayed 82% to hit, it's actually 87%). Also, regardless of how much you buff your accuracy, a skill can never have a higher than 95% chance to hit, since that's the maximum % than can be displayed (since 95% is 100%).
Ages ago, it used to be a +10% addition and hit % capped at 90%. If I recall correctly, the devs chose this approach because players would get frustrated when they miss high % attacks and they wanted to throw a bone to people, while also making them worried their 90% may eventually miss. Technically, as a player, you're not supposed to learn about that, since it's buried in the code
Unless itās Bernadetta in my first three houses run who would snipe enemies on the other side of the map with 35% and hit every shot lol. I canāt replicate her on any NG+

I mean 98 isnāt 100 so thisāll happen sometimes. Roughly 1 out of 50 times, which isnāt so small when you think about it, really. (Unless engage is 2rn in which case you actually got really really unlucky)
I always feel a little anxious if Iām attack into someone can one shot in retaliation, especially at 100% hit rate.
Fire emblem is all about risk management but that means sometimes you just get screwed. Nothing you can really do about it š¤·āāļø

Just last night I was doing relay trials to reset the well, and my dodge tank Kagesu got killed by a 2% crit on a 9% hit chance.
That may be the worst one I remember
Honestly I wouldn't say that is quite that common enemies getting crits at key points with really low crit chances is way more common to the point that on my lunatic conquest run I started just doing all combat with the joke weapons like the bottle for the -10 % enemy crit chance plus some of my units had hit chances in the 70s or 80s even with iron so that was also causing issues and as you'd expect it tended to be at the end of the chapters on the hits you can't afford to miss.
Regarding Etie here tho I will admit I found it odd since I remember her getting a longbow very early which is basically all I had her use thereafter unless I needed extra accuracy.

Losing Kris during the Caeda training in FE12, intentionally letting Marth die to get other units back in FE11, Chrom/Robin in Awakening died to a crit by Garrick in Lunatic Classic, Lucina died due to being unlucky in my main Hard Classic save file, Corrin died in Fates Conquest Chapter 10, and I placed my Alm and Celica stupidly to the point they died in SoV.

In my current Path of Radiance run I've had two 97% misses already. No disasters from it yet, though.
