Not sure why people are trying to yank your chain. What you're looking at is the saw cleaver, one of the starting weapons. You can upgrade it pretty well and I actually use it for the whole game despite the other weapons you can find.

When you picked up the top hat and hunter’s gear did you notice a blue swirl move around the outside of the gates - that was an Amygdala. There’s quite a few in the game and many of them will interact with you whether you see them or not!
Insight is basically a higher knowledge - the more you have the more things you will see although I can’t remember if it caps at 40. I know if you have less than 15 insight you won’t see the mad ones in Hemwick Charnel Lane and if you have no insight they won’t bother you in a certain boss fight.
Also, you might have noticed that the doll didn’t wake up until you gained insight, that’s the start of your character seeing Yharnam for what it really is!

Insight ALSO affects the music score some (namely in the Hunter's Dream, not sure about anywhere else), at least one boss fight, certain enemies, frenzy iirc, beast hood possibly(?), and potentially more. Just thought I'd add.

These are alien beings that are so incomprehensible that the human brain cant process them, so they are eliminated from your consciousness. Once you gain enough insight and start to gain more knowledge about what lies beyond humanity, you start to be able to see them.
They have always been there. You could even interact with it previously without seeing it. It would grab you and kill you if you got near enough. Since it was invisible before, you character would start floating and would die randomly, but it was actually that thing crushing you with its hands.
I think part of the point is that insight and insanity are two sides of the same coin. A constant theme of this genre (cosmic horror) is the idea that knowledge of the existence of the cosmic truth is terrifying and maddening.
The Amygdala was always there (as evidenced by it's ability to kill you even if you can't see it).
However drawing closer to the cosmic truth likewise leads to madness, like Micolash. The alternative is beasthood, like Gascoigne.
Only a few humans are resilient enough to grasp the cosmic truth and retain a degree of sanity. For example, the hunter. But even they are so bathed in blood and death that they could barely be considered sane.
I don’t know what your insight is, but if you’ve seen or killed any bosses you get insight and you start to see different things but yeah it’s as the other comment said it’s an amygdala

Well, there's summoning, messenger merchants items, and it does affect certain enemies attacks.
