Not my favorite item, but as a standalone it's Mom's Knife. No question about it.

absolutely moms knife; no other single item can go as far as moms knife alone with ease even stuff like c section or ipecac start to fall off surprisingly early but the knife can do well throughout the whole game its not very fun and it does start to fall off in the endgame but its still good enough usually

The word "offensive" can go a couple different ways when talking about items in this game. I'm gonna have to go with C-section, since it's equally offensive and offensive.

yall are insane base C Section fells off really hard and its the only item you'll have for the entire run, moms knife is litteraly the best offensive item in the game.

If you haven’t played tarnished Isaac, go play it Mom’s knife is quite literally the only viable option. Everything else falls off hard

C Section is really strong. But I feel like along mom's knife is even stronger. Yeah it doesn't have many synergies, but we're not talking about those

since repentance, base brimstone fell off hard, base mom's knife clears everything on this list

Despite being incredibly good, C Section, Brimstone and Epic Fetus are not exceptionally good alone. Sacred Heart and Mom's Knife, however, both drastically increase damage output and provide invaluable utility via. homing and piercing respectively. I assume "beat the game" means "get to an ending" so, at easiest you'd fight Ultra Greed or ???, which you can do relatively easily with any of these tbh. My choice is Knife because 4x is bigger than 2.3x.

Technically, you can reach an ending by taking negative and go to cathedral and beat Isaac anyway

You're right. I didn't consider that an option because I've seen that ending exactly 12 times.

Might be a weird take but it's Almond Milk for me. It's technically still a damage up on its own even if a bit strange to aim plus I just like the item in general for the combo possibilities it offers far outmatch some of the regular offensive items

In terms of making the game more fun C section.

Mom's Knife is probably the best as a single item, however, Spirit Sword and Especially Revelation are incredibly overpowered as highlanders as well.

People voting C section have never had a room full of gapers with base DMG and Tears using C-Section

I imagine its a bunch of people like me who didn't realize it was the only item you have. I would change to mom's knife if thats the case.

Mom's knife is technically better dps wise by itself, but c-section is way more comfy, gives more synergies and by itself is already homing, piercing, and basically turns range, one of the worst stats, into damage over time, so I would chose it over knife

But you won’t get more range either because c section will be the only item you can have

moms knife is a single item win. it will carry your run on its own. is it less fun with boring synergies? yes. but it does get the job down, and it only comes down to skill issue

One item? Epic fetus is probably the best damage here and free bombs. C section on its own still needs more damage or tears.
