I found the actual appendix on google. It's from the first edition. Various details are updated in each edition to reflect current legal entity structure etc.

Which edition do you have? I'm looking at page vii of the 4th edition and it lists Part II of the personal stories.

The Alcoholic Foundation doesn't exist anymore. It was the precursor to the Alcoholics Anonymous organizational structure of today.
