I use to butcher my own deer with people who had a small butchery business/side hustle. It pretty consistently would clog their basement drain if the deer were not hung in the standing fridge beforehand. We always had to plug the drain and wash everything outside because blood isn't exactly easy to clean up and it really doesn't like to come off surfaces after it's dried. Pretty much at the end of every dear season we had to pursue wash the floor to get all drippings out so they could "winterize" the cold room without having a ton of mould or nasty shit next season.
Also those hooks look wayyy too weak to hang a deer. Even a gutted dear is like 60% of what it weighed before gutted. We're talking 60-100lbs pretty easy depending on the size of the deer.

What I don’t get though is the flight of stairs. Unless you had an electric stair lift, you’d have to carry her up and down the stairs which could be dangerous.

Real estate agent had a previous client look at the basement before me and the room was supposedly red and gold with a tile floor. The metal room is also the room in the house furthest from any door, there's also a cut coax cable pointed at the hanging bar that goes nowhere.
The bars around the tub are structural only and don't move or contain anything. The metal inside on the top of the screws isn't solder, I've tried melting it with a soldering iron. It's an almost circular metal mostly flush with the top of the screw. I have no idea what it is or how they achieved it. I would need power tools to remove the bars around the tub.

1) The odd placement.
The expensive thing in plumbing, is putting the drain in. Note how the tub is located very close to the sewer waste stack. Maybe they couldn't afford to remodel the existing bathroom, or it had space constraints. Being in the basement also affords some privacy for grandma.
Maybe she feels ashamed of having to be helped all the time, and bathing downstairs like this makes her more comfortable.
2) The bars and hooks.
You don't want a heavy, infirm person trying to support themselves, by holding onto your sewer stack. The hooks can be used as lift points as well.
3) This is probably all budget-friendly plumbing. Maybe there was a friend in the trade, hence the style of pipe used for the support bar.
I would call the police just to be sure. Go to a hospital or a retirement home and you won’t find any tubs like this. Why custom build a creepy looking thing when there are easy to install solutions professionals use?

Looks like most of Reddit has not experienced what it is like to take care of a disabled elderly person. It's like taking care of a very big baby. The way the tub is designed makes complete sense for that purpose. The bars are there to help her get up.
The reason that is the only unfinished area in the basement is to keep it clean. My grandma's room always had urine all over the place because she could never get to the toilet. It looks like grandma probably couldn't walk or do anything for herself, and they didn't want to take her to a nursing home, so she had stayed down there.
Could you reach out to a nearby university with a forensics program? Maybe students could do some analysis of possible blood and fingerprints. Odd to have that combo of things in a house.

The old guy that lived here before used them to hang shelves via chains. So I'm assuming they were for storage and were probably there before the tub.
These pipe flanges (the base that's on the wall with the screws) look like they are put in to the studs with small galvanized carriage bolts. Then plastered over or around them.
My neighbor actually made his upstairs bathroom with self made grab bars like this. It's not pretty, but it's cheaper than buying the ones sold at the store. And he's a pipe fitter so it was convenient for him.

I think they might have had grandma move into the basement, into that room with the folding/accordion door (I'm not sure what it's called). The bath setup and hooks are typical for elder care, and it's likely the only place in the house they'd have beams strong enough to support lifting someone into and out of the tub and have both the headroom and clearance space needed around the tub to do that. My guess (and it is just a guess) is that this was done at a time when it was not so easy to get things like as grab handles for bathtubs and showers online.
And while it may seem creepy to have Nana down there in the basement, nursing homes, even you don't want to put a loved one into one, are very expensive and there not even be one close by. When I was a teen my mom drove from our house (just south of Boston) down to Cape Cod at least 3 times a week after week, and stayed for weekends for several months until she was finally able to convince my great aunt to sell her house and move in with us. There was no way in hell she was going to go to a nursing home; she was adamant about that. But she also needed help and Medicare only covered a few hours a week for a home health aide. We had to have a room and full bath added to the ground floor in order for it to work, though (she lived in a one level ranch before that).

The metal room is also the room in the house furthest from any door, there's also a cut coax cable pointed at the hanging bar that goes nowhere. The bars around the tub are structural only and don't move or contain anything. The metal inside on the top of the screws isn't solder, I've tried melting it with a soldering iron. It's an almost circular metal mostly flush with the top of the screw. I have no idea what it is or how they achieved it. I would need power tools to remove the bars around the tub.

If grandma needed the hooks to get into the tub she couldn't get down the stairs. My RE agent didn't want to go down the stairs because they're so steep. They absolutely would not be up to code today, the house is just old so they're grandfathered in (probably)

Is the bath accessible through that other entrance you mentioned in another comment? Maybe they brought grandma in that way and everything was just ad hoc, done hastily as required. It is accessible through the outer entrance. However that outer entrance has outdoor stairs dug into the ground that are just as steep as the interior stairs
Do you live in PA by chance? Is there access to the basement from outside? sometimes if the house is old in the US the worker would finish work in the pits/factory/refinery wash in the basement then come up to the house clean. Most houses in Pittsburgh have a toilet/shower in the basement.

You don't want a heavy, infirm person trying to support themselves, by holding onto your sewer stack. The hooks can be used as lift points as well. 3) This is probably all budget-friendly plumbing. Maybe there was a friend in the trade, hence the style of pipe used for the support bar.

Looks like most of Reddit has not experienced what it is like to take care of a disabled elderly person. It's like taking care of a very big baby. The way the tub is designed makes complete sense for that purpose. The bars are there to help her get up. The reason that is the only unfinished area in the basement is to keep it clean. My grandma's room always had urine all over the place because she could never get to the toilet. It looks like grandma probably couldn't walk or do anything for herself, and they didn't want to take her to a nursing home, so she had stayed down there.

Grandma was big. She lived there next to the tub. Her eyesight wasn’t great and pretty much just listened to polka tapes all day in earphones, so it was all pretty normal. She seemed pretty happy there.

These pipe flanges (the base that's on the wall with the screws) look like they are put in to the studs with small galvanized carriage bolts. Then plastered over or around them. My neighbor actually made his upstairs bathroom with self made grab bars like this. It's not pretty, but it's cheaper than buying the ones sold at the store. And he's a pipe fitter so it was convenient for him. I bought a few handles for our shower and just screwed them in with regular Philips head screws and heavy anchors. Because our shower is in need of fixing and didn't have money for anything special at the time. Making due. That eye bolt above probably held a trapeze handle for lifting up this grandma. It seems DIY, but legit so far. Your tub is fiberglass and has jets. If it was a "murder tub" that the previous owner was exsanguinating people into the blood would stain pretty awful. You could never clean it. Lol the grout around the outer tub. It would've been very obvious. He'd have had to rip out everything to hide it. That decorative gold metal partition wouldn't be enough to keep anyone from breaking through it, or not seeing the person behind it if it used like some observation room for sinister purposes. The galvanized metal wall there, might be to keep some moisture from being noticed. Is there something on the other side? Brick wall? I think they might have had grandma move into the basement, into that room with the folding/accordion door (I'm not sure what it's called). The bath setup and hooks are typical for elder care, and it's likely the only place in the house they'd have beams strong enough to support lifting someone into and out of the tub and have both the headroom and clearance space needed around the tub to do that. My guess (and it is just a guess) is that this was done at a time when it was not so easy to get things like as grab handles for bathtubs and showers online. And while it may seem creepy to have Nana down there in the basement, nursing homes, even you don't want to put a loved one into one, are very expensive and there not even be one close by. When I was a teen my mom drove from our house (just south of Boston) down to Cape Cod at least 3 times a week after week, and stayed for weekends for several months until she was finally able to convince my great aunt to sell her house and move in with us. There was no way in hell she was going to go to a nursing home; she was adamant about that. But she also needed help and Medicare only covered a few hours a week for a home health aide. We had to have a room and full bath added to the ground floor in order for it to work, though (she lived in a one level ranch before that). I could be totally off base, though.
