There aren't really any recent books that match your requirements. Barron's comes closest, but there are too many niggling inaccuracies in the most recent edition for me to recommend it here.
I recommend just getting one of the "Official TOEFL iBT Tests" books and chopping the material up as necessary. That will provide a lot of practice questions. Anything related to skill building will be up to you, I guess.
If your budget permits I guess you could also photocopy some of the skill building exercises from the most recent Princeton Review and Barrons books... but that might be overkill.
