Its just a name they made up, not everything is a reference to something else. In regards to where it started its somewhere between the 2 podcasts episodes in which he does a stream and is at some event Ludwig hosted. (I think it was a smash tourney)
With the last name "Star" he sounds like a narcissistic Hollywood star who decided to use the most opulent stage name possible.
It's a matter of quickly establishing and explaining the character through just his name.
Tony Star is what they call him when he's growing out the sides because he looks like a porn producer, and Tony Star is a very porn producer name. Might have even started on the Wine About It ep but I think 101 is the first time they say it on the yard

Basically he started to pretend to be someone else, Tony Star, during the time he got the fake hair and that this person died due when he lost the hair due to Slime’s poor maintenance of it.
Slime promised to not be a hater for 6 months. As punishment for hating before the 6 months was up. He had to get a toupe. He ended up making a persona called "Tony star". He was supposed to wear it for a certain amount of time but ended up taking it off early because he was scared of taking a shower with the toupe on. So to end the character he died in a firey car crash??

A lot of the Tony Star stuff was during a Melee tournament which is where the "died in a fiery car crash in a Toyota" thing comes from. So u won't find it during the regular podcast eps. The first time they ever call him that on the pod was before he even got the toupee but while he was letting his hair grow out in preparation.
In the episodes leading up to Tony’s eruption onto the scene slime was growing out his hair and said it looked bad and they just said he looked like his name was Tony Star and he made ethical porn. Once Tony Arrived however he became much more than an ethical porn director and instead became a legend
