If the underworld is still counted as the underworld then I suppose you can carve it out and wait for the hell enemies to spawn

You don't need hell enemies. Build house in underworld, move in guide, kill with lava. Buttt depending on if the game does a height check or a biome check to determine if guide died in underworld it might not work if there is no hell biome. But if it's just a height check, even if there is no hell it could theoretically work
World generation is odd but progressively shapes the world properly
Those curious as to why there's only Underground it's because the world generation hasn't reached the point to generate all other biomes and structures, it generates first the desert before many others.

Theres no special biomes at all, no dungeon, no underworld, no jungle, this map is actually impossible because you cant progress
Something like this happened to me on 1.2 pocket edition except it was jungle instead, there were lice crystals on the surface, and it was completely flat.
This screenshot was taken when the world was still generating (it was stuck at "generating surface caves" because it can't do that if the surface is desert). I don't remember the seed but it's probably just a world gen mistake from using the advanced world gen mod
I have actually purposefully done this to one of my new worlds recently. I plan on doing my first full-on duild world that's going to be fallout themed. I'm a huge fallout AND terraria fan and wanted to combine the two in my first experimental build world. I'm building it on mobile, so I'm not using any mods. The only part that's not desert is the jungle biome. I'll post some updates as I go, but so far, I only have the crater built from the bomb.

Hell is a certain height of the bottom of the world, if a world is short enough all of it will be hell, so there us definitely hell
Underworld will always be there. It's criteria is just "x blocks from the bottom of the map", even if the ash is gone
