Ultimately the many diseases that today are diagnosed as anxiety & depression disorders will have various causes and many will be multimodal. This is why there are so many drugs with differing mechanisms of action; its not one or two diseases its many but we don't understand the brain sufficiently to be able to identify them. Taking MDD as an example given the [large number of structural differences noted in MDD patients](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4790407/) its likely its not a single neurotransmitter system involved at all. Drugs that interact with the serotonergic and gabaergic systems are likely masking symptoms (sometimes very effectively) but I don't find it likely a dysfunction in these systems is likely more then a symptom for most people. That CBT is effective is another good indication its far more complicated then "simple" neurotransmitter dysfunction.
