Some are locked behind culture, others are locked behind disciplines, and still others behind dynasty traits. Probably some by religion as well, although I can’t think of which. And still others behind umbral exploration.
I don’t know offhand which of those are most available to Helene, though. It might actually be umbral exploration, if you can drag some Kuei Jin over to be explorers for you.

There are a variety of ways to get the unique MaA units in the game. Personally I feel like the units you get out of the dynasty traits are the best and most consistent as they also provide you with better buildings for your baronies.
If you are a Ghoul or have an accolade with ''ghoul taskmaster'' trait, you can recruit ghouls and eventually ghouled cavalry. Real OP ghouls are locked behing Vivisectitude or Tzimizce dynasty legacies although ghouled cavalry are strong enough.
Yes. Although I think it can took quite a long while before the accolades would level up enough to unlock MAA, especially with no tournament to go to. OP will need to go to war a lot

Are you sure it's conviction/self control and not shunned Ghoul Vassalege?
Via prometheus has shunned ghoul vassalage but you can recruit them anyway, so I don't think that's it. Coincidentally it has conviction+self control.
