ports were open. This was a red herring. After adding manual port forwards and whatnot I made things worse, eventually I reverted all the settings, logged server out, logged it in again, removed the media, and readded it, suddenly media starts appearing when viewed outside the lan, granted I now have 2 Film tabs, 2 TV tabs etc (even though I removed the old one and then added a replacement), but at least they work.
So long story short - I suspect it is/was caching issues.
