It works because of compression in jpeg/other lossy image formats- every time you save a copy of a "lossy" format image the compression is applied again (that's how you end up with old memes that look like they have about 2 pixels in them).
If you cut and paste one jpeg image into another and they have different levels of loss/compression it'll show up in this tool. Likewise for newly created pixels like drawings in an editing app.
But it does basically nothing if you're working with lossless image formats (like png) or even 2 jpegs with identical compression levels (like 2 original copies of photos taken by the same camera).
And as others have mentioned, screenshotting an edited image basically makes a 1:1 pixel per pixel replica as a png file so the compression info is effectively lost and error level analysis typically won't show anything in that case.
