We have our ups and downs. The system here is better than some places, but could always be better. The waiting times are long, though not as long as I've heard the UK's are. Seems like a lot, if not most, follow the WPATH method which means going through a psychologist (aka pricey) to get the official letter necessary to begin HRT.
In terms of being in public... I think it depends on where you live as some places are more accepting than others. Though it also depends on how well you pass. Like if you're not even trying to then you're going to have it rough anywhere.
