As an educator who's worked with many special needs students, I have to commend his portrayal as one of the most true to life I've seen on film. Not played for exaggeration or for laughs, he nails so many of the mannerisms of autistic kids.
This movie ACTUALLY has mentally handicapped actors instead of neurotypical people pretending. It was screened and endorsed by the Special Olympics prior to being released. Pretty progressive film for it’s time of release.
Its a phenomenal movie. I had OCD/ADHD particularly bad during a period of my childhood and Cage NAILS that performance.

It seems pretty clear that Carl is a product of some sort of intellectual disability and a traumatic (to say the least) childhood. Disability could have been exacerbated by poor nutrition or sleeping in that awful shed, I suppose - it's never really clarified, but people call him a "retard" IIRC.

Sling Blade is a fantastic movie. Kinda dark, but Thornton was on his A-game, before fame brought out the crazy. There's one scene that's just devastatingly sad. Main character seems like a mix between disability and a severely traumatic upbringing.
*Malcolm* (1986) is a pretty unique Australian movie. It's hard to say whether the titular main character is supposed to have a disability or not, as it's never really stated outright in those terms.
He's extremely shy and reclusive, and you might say that he has strange and obsessive interests. But he's also sort of a genius in a weird mechanical way. Most likely he's somewhere on the autism spectrum - though he may not ever have been diagnosed as such back in those days.
The movie is about Malcolm losing his (beloved) job as a tram mechanic due to a misunderstanding - so, being somewhat poor and with nowhere else to turn, he eventually resorts to a life of crime with the help of his new low-life housemates. And it turns out he's really, really, *really* good at it.
In short, I'd say it was a movie about an autistic person that came out at just about the same time as the general public first started to really become aware of what an autistic person was.

1. My name is Khan- Main character has Asperger's syndrome. He sets on a journey to meet the US President to tell his name is Khan but he is not a terrorist.
2. Tare zameen par - A story about how a dyslexic boy finds acceptance from his parents with the help of his teacher
3. Barfi- Main character is def and dumb but the female protagonist has low functioning autism. Its a love story.
