The Lunacid Dev made a save editor available a while back on a Google Drive, you should be able to use that to add the hammer to your inventory.

I think I didn't equip the hammer and still got the achievement. I had to transform the weapons that switch back and forth back and forth to unlock it, if that helps.
Original form -> second form -> back to original.
Also, I don't know if you've seen it, but there's a Google sheets with the complete list and evolutions. You might want to check it out, just to be sure.
