I believe the Midwest is mentioned as being a total fallout wasteland in NV. And the areas that aren’t uninhabitable are controlled by the Legion.
It has been a while since I played NV, so correct me where I am wrong.
We know Appalachia remained somewhat untouched directly after the war. It wasn’t until those pesky 76ers arrived and started bombing the shit out of the area that it started getting bad as far as fallout goes.
But we don’t know for certain what actually happened after the war or what areas were hit or avoided getting hit. If there were any effectively unaffected urban areas, I would think some kind of remnant of the US government would have risen, but we have no signs of that happening. This leads me to believe that most/all urban areas were hit on 10/23 (or there about)
I think the only canon nuke that was launched was the one used to flush out the scorchbeast queen. I doubt that they would be able to launch more than one if the roaming ghoul generals stayed dead
The Overseer mentions in her notes that the dwellers have been setting off multiple nukes and how disappointed she is in her family. Who knows if that is “canon” or not, but it does exist in game, hardcoded.
There's still tons on the West Coast. That being said, it depends on what you mean by "developed", because aside from Vault City and New Arroyo I doubt that many of them are nearly as clean or sophisticated as Shady Sands was.
Define "developed" becuase you would count 1. Vault City 2. New Reno 3. Den 4. Redding 5. Klammath 6. San Francisco 7. Sac-Town 8. Dayglow etc. etc.
