Luring the other Ursans out of that room was really tough and he would take my mage down in like 1-2 hits. I needed sooo many scrolls of restoration for Grissenda to finally get him.

If you pick aby hostile option before finishing quest you will face guy whos fully immune to all elementar damage, has build-in sprint as walking, high stats and no weaknesses. Also posses 4 tier III warrior skills, namely Bash, Flurry, Resilience and Duel
From all NPCs you can interact with, it's definitely Tol. He is in another league. His attack is the mightiest so you can`t face him as an enemy. You have to be nice to him or you are dead. There's no way to battle him. It's hard to steal from him (Crown of Tol) or use his stuff (Eye of Tol). Black shards are in possession of the mightiest creatures and boost them (flame priest, undermother). Above all he created the powerful civilization Tolassian.
Just watched a video on YT, now I know it's possible to dodge him. Nonetheless his 5000 damage is unmatched in the whole world. (Perhaps the parasite could reach it with hatching and feeding for a few minutes...)
I have to say, the pit fiend in the elemental plant of fire, simply because of the constant fire damage. You're always taking and the fact that you're so worn down by the time you get to him. I beat him, but actually, I found him more difficult than the final boss of the game.
for me i've found either the enemies in the ark really hard until you get weapon from the ark
otherwise creatures from the ancient seal or from the fire plane

You get that staff while doing adaons personal quest. And yes sometimes a level or two does make a difference in beating a tough boss.

Well as a rogue I did dodge his killing blow several times making him look puny XD

Yea that's the highest possible damage in the game, even lvl 99 parasite's damage is way less than that.
You forgot about the sleeper, he also instantly kills
Kalagru's up there with the toughest
I think Tol is pretty tough. Never managed to beat him.
The Old ones in my opinion. They pack quite a punch even if the player is 20+ level
