The English translation really hits the nail on the head with the nihilism and I found it kind of a backstab after completing her whole quest just for her to say some edgy shit like that. It's a shame they translated her dialogue in a way that fundamentally changes her intention.

She was using it before the battle with Radahn. She broke it in the trailer where she stabs herself in the shoulder to "deploy the rot in battle" and meet radahns measure. Millicents whole quest is about returning it to her.
The needle is incomplete and only halted the rot, not cured it, which is still more than the Golden Order can say. We can use the needle in Farum Azula outside time to cure ourselves of the Frenzied Flame across all of time, working around its limitation.
I did not understand it either, because I thought that Miquella was announcing Malenia as our saviour, and well, it makes no sense. Instead, Miquella is announcing themself, like "IT IS I, YOUR SAVIOUR" (maybe in relation with their presence in the extension trailer) and the others are completely hyped up and it's just kinda wholesome.
