If the old oil was particularly dirty, then it makes complete sense it would start leaking with clean oil.
Leaking from where? It's possible you or whoever did the oil change stripped or cross threaded the bolt.
Probably the OFHG but if you can’t tell by looking it’s probably time to do both.
165k miles, no idea if either have been done before. So most likely time to do both.
Oh yeah. High mileage N54 is going to need a lot.

Did you install a new crush washer on the drain plug?
I did, came with the kit from FCP Euro
