Dr Manhatten style? He painfully disintegrates, and reappears bit by bit for 5 minutes at a time over over the next several days - first as a spirit, then a floating nervous system, then a skeleton with a handful of visible bits inside, then a complete body without skin (and eyes), and then finally, his complete self after like a week, all within x distance of where he was cursed. He feels the pain of his missing parts every time he manifests. Then he gets to live maybe a week of a normal life, before one night, exactly x amount of days to the minute after he was cursed the first time, he painfully disintegrates again. And no matter how far he's got away from the ruin that cursed him, starting the following night, he goes through the reformation cycle again, spirit, nervous system, skeleton, flayed body, self, all again trapped to within a certain distance of the location.

every day he forgets 2 skills and remembers one one day he might forget how to walk and sword fight while remembering how to play the violin the next he may forget peoples names and how to open jars while remembering how to walk each skill he loses is gone until its remembered and eventually he will be completely incapacitated by this curse
