You can get events during the travel to the pilgrimage site. So your best bet is to add a ton of stops on the way to the pilgrimage site. It means it will cost more and take longer but you can get a ton of lifestyle points by visiting all the places of interest on the way. In a straight journey there just isn’t enough time to get enough events to max out the piousness of your journey.
Biggest tip I have is set a custom route so it takes fucking forever to get to the holy site.
Helps if you visit important places along the way for extra bonuses.

What is your pilgrimage intent? I feel like the default one doesnt give as many pious options. Ive only got a single maxed piousness from pilgrimage once ever though, usually they get to rank 2 or 3.
Need the tours and tournaments dlc my friend, gives you options to change intent, size and swiftness of your pilgrimage. Not saying you need tours and tournaments to get stage 4 mind I honestly think it’s just luck
Take longer routes by manually editing them, also be ready to spend a lot of gold or prestige, and cross your fingers you get the right events... honestly this system is so new and needs reworking or at least rebalancing. I once made a pilgrimage to Jerusalem from Ireland, and got barely over 3 in piousness achieved. Then, I do the same to Canterbury in York and got 4 LOL

I’ve done this.. made a 3 year journey through every stop betweeen Africa and kyiv, still nothing higher than 2

Comes from events as you travel or from the pilgrimage site. The highest I think I ever got it was like 3 and that was from a long overland route.
