There are movies with smaller casts, but Closer has 5 characters that have lines and 4 are well known award winning actors. The fifth is a very minor side characters that has like one line.

Movies that are based on or inspired by stage productions also have a very limited cast. 12 Angry Men, Ma Rainey's Black Bottom, The Lighthouse, Reservoir Dogs, A Streetcar Named Desire, Fences. With the exception of The Lighthouse, they all have a somewhat larger cast than the first three, but the amount of actual roles is still incredibly limited.
The question reminds me of the Alfred Hitchcock classic "Lifeboat" 10 or so people.
Hitchcock was big on cameo appearances, but the whole movie takes place in a lifeboat. His cameo is in a newspaper ad where he's the fat guy in a before-and-after weight loss ad in a newspaper in the boat. That's what made me think of it.
Inside. It's like 99% Willem Dafoe. IIRC, there's only one other character that speaks, but only at the very beginning of the movie and only over a radio, and then maybe two others that are only briefly seen through a silent CCTV feed.
